  		Family wedding photo of Elaine Forman and Maurice Tobias This is a family photo from the wedding of Elaine Forman and Maurice Tobias-June 29, 1952. Also pictured are Sarah Katzen Forman, Morris Forman, Leonard Forman, Rosalind Michelson Forman, Robert Forman, Fran Forman and Sadie Karpa Tobias. There are other relatives pictured that are from the Tobias side of the family.
  		Four Generation of Forman's. This picture was taken sometime in late 1979. It is four generations of Forman's. Standing on the left is Robert(Bob) Forman and his father Leonard Forman on the right. Seated is mother, grandmother and great-grandmother Sarah Katzen Forman, holding a newborn Rosha Forman.
  		Morris Forman and Sarah Katzen Forman On the back of this picture it says "Sarah Forman 21" Not sure whether this means that it was taken in 1921 when she was 23 years old or whether she was 21 years old at the time of this picture. It also says Garden Wing.
  		Sarah Katzen Forman Date unknown. This possibly is Sarah's picture that was on her driver's license. She never drove a car and decided at the age of 60 to get her license.

  		Marriage Certificate-Morris Forman and Sarah Katzen This is the marriage certificate of Morris Forman and Sarah Katzen. It was filed on June 6, 1919. It appears that they were then married on June 8, 1919. On this certificate, it states Sarah's name as Sylvia Forman. Throughout my life, I had never ever known her as Sylvia, nor did any of her relatives or friends refer to her as Sylvia. It must have been a clerical error? Or were they never legally married???? Then this means that there is certainly a great deal of illegitimacy within this family tree!!

Sarah received this award in recognition of outstanding volunteer service to Levindale and its residents. She volunteered at Meals on Wheels every week for years.
